Disabled Parking Permits & Plates

We offer services for individuals with disabilities who need parking permits or specialized license plates. Below you’ll find information on eligibility, how to apply, fees, and renewal requirements.

 

Disabled Person Parking Permits


Who Can Apply?

  • Florida residents with a temporary or permanent disability.
  • Organizations that regularly transport disabled individuals.


Required Documents:

  • Completed Form HSMV 83039 – Application for Disabled Person Parking Permit.         
  • Completed Form HSMV 83040 – Application for Expectant Mother Parking Permit        
  • Signed by a certifying medical authority within the last 12 months.
  • Valid Florida Driver License or ID Card, or a clear copy.
  • Original signatures from both applicant and medical authority.


Applying Through a Representative:

  • A representative can submit your application in person or by mail.
  • They must provide your completed application and a copy of your ID.

 

Approved Certifying Medical Authorities:

  • Florida-licensed physician (MD/DO)
  • Chiropractor
  • Podiatrist
  • Optometrist (for vision impairments only)
  • Advanced Practice Registered Nurse (APRN) under a licensed physician
  • Physician Assistant (PA)
  • Out-of-state physician (must provide proof of licensure and sign Form HSMV 83039)

 

🟦 Permanent Parking Permit (Blue Placard)

  • For individuals with long-term mobility issues or who are legally blind
  • Valid for 4 years, renewed by the permit holder’s birthday
  • No fee
  • Requires a new application (Form HSMV 83039) for every other renewal
  • Limit: 2 placards per person

 

🟥 Temporary Parking Permit (Red Placard)

  • For individuals with short-term disabilities
  • Valid for up to 6 months, as certified by your physician
  • Expectant mothers may receive a 1-year temporary permit 
  • $15 fee
  • If an additional permit is needed within 12 months, no extra fee

 

Replacement Permits

  • If your permanent permit is lost or stolen:
    • Submit Form HSMV 81346 (Application for Replacement)
    • If it's been more than 12 months, a new Form HSMV 83039 is also required

 

Using Your Permit

  • Must be used only by the permit holder
  • Must be displayed from the rear-view mirror when parked
  • Must not hang while driving
  • Must have:
    • A yellow registration decal on both sides
    • The permit holder’s driver license or ID number on one side

 

 Wheelchair License Plates


Eligibility:

  • For those with a long-term mobility impairment or who are legally blind
  • Must be registered in the disabled person’s name

Fees:

  • $28, plus any applicable registration fees
  • Renewed annually on the owner’s birthdate
  • Medical certification must be renewed every 4 years

Eligible Vehicle Types:

  • Private-use cars and light trucks (≤ 5,000 lbs)
  • Passenger vehicles (under 9 seats) for hire
  • Motor homes or truck campers (non-commercial)
  • Trucks with a gross vehicle weight under 8,000 lbs

 

Disabled Veteran License Plates


Veterans who are 100% service-connected disabled are eligible for a free Disabled Veteran license plate.

Requirements:

  • If requesting a Disabled Veteran Wheelchair plate, Form HSMV 83007 is required     
  • Provide proof of: 
    • 100% disability rating from the VA
    • Or that the vehicle was purchased with VA financial assistance
